Midwest DERMaesthetics Membership Program
Membership Terms & Conditions
By enrolling in a Midwest DERMaesthetics Membership, you agree to the following terms and conditions:
- Members may choose from three membership tiers: Custom Facial, Dermaplane Facial, or Deluxe HydraFacial. Each plan includes a monthly treatment and unique perks, as outlined in the plan details.
- Membership includes one service per 30-day billing cycle. One unused service may roll over to the following cycle, but it must be used within that billing period or it will be forfeited.
- The initial term of membership is three months. After that, the membership automatically renews on a month-to-month basis until canceled.
- Membership fees are automatically billed to your card on file starting date of enrollment followed by the first (1st) of each consecutive month during membership. All payments are final and non-refundable.
- Services included in the membership are non-cumulative, non-transferable, and may not be exchanged, banked, or shared.
- Additional services not included in your membership may be purchased at regular, non-member pricing.
- Members may upgrade their membership after completing one full billing cycle. After upgrading, members must remain at the new level for at least three billing cycles.
- All services are subject to professional evaluation. Our team may decline or modify a service if it’s not medically appropriate or advisable.
- Members may cancel anytime after the initial term with 30 days’ written notice. Notice must be provided via email or mailed to the address listed in your agreement.
- Memberships may be frozen once per 12-month period, for a minimum of one billing cycle and a maximum of three. A freeze fee may apply and approval is required.
- Missed appointments or cancellations with less than 24 hours’ notice will result in a $50 fee, charged to the card or account on file.
- Members receive 10% off all in-office professional skincare products. This discount cannot be combined with other offers or used online.
- Each member receives $50 in Birthday Bucks to spend on services or products in the med spa during their birthday month.
- Members under the age of 19 must have written parental or guardian consent. A parent or guardian must remain on-site during all appointments. For members 15 and under, a parent or guardian must also be present in the treatment room during each service.
- All members must follow Midwest DERMaesthetics policies and conduct guidelines. We reserve the right to suspend or terminate memberships due to noncompliance or inappropriate behavior.
Membership FAQ
How do the memberships work?
When you sign up, you’ll choose from three plans—Custom Facial, Dermaplane Facial, or Deluxe HydraFacial. Each includes one monthly treatment, plus exclusive perks like product discounts, member-only promotions, and birthday rewards.
Is there a minimum commitment?
Yes, there’s a three-month minimum to start. After that, your membership renews month-to-month and can be canceled anytime with 30 days’ written notice.
What if I miss a treatment?
You can roll over one unused treatment to the next month, but it must be used within that billing cycle or it will be forfeited.
Can I cancel anytime?
Yes—after your initial three-month term, you’re free to cancel with 30 days’ written notice (email or mail is fine).
Can I freeze my membership?
Yes. You can freeze your membership once every 12 months for 1 to 3 billing cycles. A freeze fee may apply, and approval from our team is required.
Can I upgrade my plan later?
You sure can. After one full billing cycle, you’re eligible to upgrade your membership. After upgrading, you’ll need to stay at the new level for at least three billing cycles before making another change.
How does billing work?
Your membership fee is charged for your first membership treatment when you enroll and then automatically charged on the 1st of the month going forward . All payments are final and non-refundable.
Do I get discounts as a member?
Yes! Members receive 10% off all professional-grade skincare products purchased in-office, plus access to members-only promotions. You’ll also receive $50 in Birthday Bucks to use on services or products in the med spa during your birthday month.
What happens if I cancel or miss an appointment?
Please give us at least 24 hours’ notice to cancel. Missed appointments or cancellations made with less than 24 hours’ notice will result in a $50 fee.
Can anyone become a member?
Yes! However, members under 19 must have written consent from a parent or legal guardian. A parent or guardian must remain on the premises during all appointments. For members 15 and under, a parent or guardian must also be present in the treatment room during each service.
What if a service isn’t right for me?
Every treatment is subject to evaluation by one of our licensed aestheticians—and, if needed, a physician. We reserve the right to modify or withhold services if we determine they aren’t safe or appropriate for you.
Can I share my membership with a family member or friend?
Memberships are non-transferable and may only be used by the person enrolled.
Will I be reminded before my card is charged?
No monthly reminders are sent. Charges occur automatically on the first of every month, so we recommend making a note in your calendar.
Can I use my Birthday Bucks on products?
Yes! Birthday Bucks can be used on both services and in-office product purchases during your birthday month.
Do you offer gift memberships?
Not at this time—but keep an eye out for future promotions and seasonal specials that make great gifts!
