Today’s healthcare climate focuses on excellent communication between physicians and patients and at Midwest Dermatology we take that requirement seriously.

That’s why we want you to connect with us via the Modernizing Medicine Patient Portal we’ve created for your account. Why take the time? Because our portal provides important tools to ensure your best care:

Connect to your electronic medical record and find:

• Visit notes with your impression (diagnosis based on exam) and treatment plan
• At home regimen recommendations
• Recent product purchase history
• Test results from integrated labs, without calling or waiting to hear from our office (not all labs integrate at this time)
• Date and time of your next appointment, along with your appointment history.

Update us regarding changes to:

• Address, phone #, email and any demographic / contact information
• Changes to insurance
• Allergies, health conditions and alerts and all health history important to your dermatology care

What’s best is that this can all be done via your phone, tablet or desktop, around the clock. And by updating before your next appointment, you will save valuable time in the office while ensuring your provider has accurate information needed for your best medical care.

How to Create / Log in to your Patient Portal

• Click on the link below
• You will be prompted to enter your date of birth and last name.
• You will then click on a link that will send you an email with set up instructions.  Check your email to choose a password and activate your account.  Note:  This email link will expire in 72 hours. 

*Please note, while you can view your patient portal conveniently from your phone, it is best to add and change information in your portal using a desktop or tablet/ipad as the information may not save from your phone.

Once you are in, navigating is simple! Remember to jot down your user name and password for future log-in and check back with us for any updates on content available through your portal.
